What to Expect Your First Visit

Examination

The UND Center for Family Medicine is a clinic that serves the general health care needs, including preventive care as well as urgent problems of adult and pediatric patients. It also serves the purpose of teaching and educating our future internal medicine doctors.

At your first visit, you may be seen by both a resident physician (a doctor who has finished medical school and is in residency training) and a faculty physician who will supervise and teach. Faculty physicians have a specific time commitment required to provide both direct patient care and clinical teaching/supervision of the residents.

Paperwork

When visiting the UND Center for Family Medicine for the first time, we ask that you arrive about 15 minutes before your appointment to fill out paperwork. Please bring your insurance card and be prepared to answer some brief health history questions.

For added convenience, you are welcome to fill out the Pre-Registration Form and review our Privacy Practices prior to your visit.

New patients without insurance are expected to pay a $25.00 co-pay up front. When this is not possible, patients can discuss the account with the Business Office following the appointment to make arrangements for payment. Call 751-9505 regarding any health insurance and/or billing questions.

Written consent is required when someone other than parent/guardian will accompany a minor patient to their appointment.  If it is anticipated that the parent/guardian will not be present for the appointment, the Consent for Minors Medical Care and Information should be completed by the parent/guardian and be brought to the appointment.
